Analysis of Safety Culture Weaknesses in Chemical Safety Board Investigation Reports

The U.S. Chemical Safety Board (CSB), an independent, non-regulatory federal agency that investigates the root causes of major chemical incidents, has firstly analyzed safety culture as important element to maintain process in investigation report “BP America Refinery Explosion” 2007. On same year, Center for Process (CCPS) published Risk Based (RBPS) which was newly added element. author found six CSB reports weaknesses and related them essential features RBPS. Discussions are made on results relations.
